The Prader Orchidometer is a tool designed to assess testicular volume. It features a series of 12 beads, each numbered and increasing in size from 1 to 25 ml.

These beads are used to compare against the patient's testicles, allowing the volume to be determined by the bead that most closely matches in size. Pre-pubertal volumes (yellow) range from 1 to 3 ml, pubertal volumes are 4 to 12 ml (orange), and adult volumes are 15 to 25 ml (red).

Variations in testicular size compared to other maturation indicators can provide important insights into various conditions. Smaller testes may suggest primary or secondary hypogonadism. Testicular size is also useful in differentiating types of precocious puberty. As testicular enlargement is often the initial sign of true puberty, the orchidometer is frequently used to confirm the onset of puberty in boys experiencing delays. Enlarged testes (macroorchidism) may indicate fragile X syndrome, a leading cause of intellectual disability.
